Co-creating a model of care with Central Health - from hospital to community

The Tsao Foundation is a member of Central Health, a network of service agencies partnering Tan Tock Seng Hospital in its endeavour to develop an integrated model of care for residents in the central zone of the National Healthcare Group health cluster.

We hope to advocate for a community-based care which is seamless, person-centred and of quality that is affordable by everyone and anyone.

The Foundation also aims to support the initiative through efforts in workforce transformation and healthcare training.
