Care Management


Most people are unfamiliar with the services and resources in the community that are available for their support. Hua Mei Community Care Management Services (HMCCMS) works with frail and at risk elders (and their caregivers, if any), to advocate, link up and monitor a support system of medical, homecare and social services to enable them to live at home in optimal health. HMCCMS was started in 1998 in a pilot project to launch care management in Singapore. It is the longest running, continuous service of its kind in the country.

 

Approach:
Unlike other agencies, HMCCMS offers care management that is team-led by nursing and social work professionals. Through that and the access to further medical and pyscho-social services within the Hua Mei Centre of Successful Ageing, we are able to comprehensively support the optimisation of the client’s health and wellbeing.

Using a person-centred approach, we aim to ensure that the client has access to the right service at the right price and time, and that his care plan is sustainable within the community and optimises his health and wellbeing.

We provide:
thorough and holistic assessment of the older person's medical, social and emotional needs, strengths and resources
information and advice on relevant services/resources to meet client’s goals, enhance his capacity for self-care, and optimize his wellbeing in the community
viable, sustainable care plans developed in collaboration with the elder, his family and community care partners to support his living at home
linkage, coordination and monitoring of needed services such as physical rehabilitation, home help, and financial assistance, and ensure that the condition of the elder person is stabilized
   
We serve persons who are:
aged 60 years and above
having difficulty managing multiple medical, social or psycho-emotional needs
residents of Bukit Timah, Bukit Merah, Buona Vista, Chinatown, Commonwealth, Clementi, Outram, Queenstown, Tanjong Pagar, Telok Blangah, Tiong Bahru.
