Overview

The Hua Mei Elder-Centred Programme of Integrated Comprehensive Care (EPICC) is a day club programme that supports older persons aged 60 and above who wish to remain engaged in the community despite multiple chronic medical conditions, physical frailty, cognitive impairment, or limited family and social support.

EPICC provides person-centred, multidisciplinary care that addresses each individual’s medical, social, emotional, and functional needs in a warm and supportive environment. Every senior receives a personalised care plan based on a comprehensive assessment of their health, wellbeing, and social circumstances.

Our centres in Tiong Bahru and Whampoa are co-located with other services, including primary care, care management, counselling, rehabilitation, and social activities. This enables older people to conveniently access integrated support that promotes wellbeing, independence, and quality of life.

Launched in 2011, EPICC was piloted through a randomised controlled trial to evaluate its medical and social impact. It was among the first programmes in Singapore designed to support older adults with complex care needs, including those without dedicated caregivers at home. Today, EPICC continues to serve some of the most underserved seniors in the community.

The programme is modelled after the internationally recognised Program of All-inclusive Care for the Elderly (PACE) and is partially funded by the Ministry of Health.

Our Programmes

Maintenance Day Care

For older adults who require supervision, social engagement, health monitoring, and on-going care needs while continuing to live in the community.

Dementia Day Care

For persons living with dementia who would benefit from structured activities, cognitive stimulation, social interaction, and specialised support in a safe and supportive environment.

Centre-Based Nursing

Seniors benefit from nursing services such as medication management, health monitoring, and wound care, helping them maintain their health while continuing to live independently in the community.

What We Offer

On each programme day, seniors spend 9:00 am – 3:00 pm on weekdays at the Centre and benefit from:

  • Group maintenance exercises to support strength, mobility, and overall wellbeing
  • Structured and engaging activities tailored to individual abilities and interests
  • Opportunities for social interaction alongside counselling
  • Nursing care, including medication management and basic nursing procedures
  • Care management, coordination and monitoring by a multidisciplinary team
  • Transport services (where applicable)

Regular attendance enables our team to closely monitor seniors’ physical health, emotional wellbeing, cognitive function, and social needs, allowing care plans to be reviewed and adjusted promptly as needs evolve.

Personalised Case Management

Every senior undergoes a comprehensive assessment that considers their bio-psycho-social needs. Our multidisciplinary team develops and reviews a personalised care plan, while a dedicated group chat involving the centre manager, social worker, nurse, and the senior with their family members or caregiver helps facilitate ongoing communication, care coordination, and timely support as needs evolve.

Integrated Multidisciplinary Care

EPICC is supported by a multidisciplinary team comprising doctors, nurses, social workers, physiotherapists, occupational therapists, centre managers, programme assistants, administrative staff, and drivers. Working together, the team delivers seamless, coordinated, and holistic care tailored to each senior.
