Seniors should never be limited in their access to healthcare because leaving home is difficult. Hua Mei Mobile Clinic provides nursing-led, team-based home care for homebound seniors, ensuring they continue to receive timely, holistic care to maintain health, dignity, and quality of life.

Our Holistic Care Model

Our care is built on a biopsychosocial approach, delivered by a multidisciplinary team:

  • Doctor – Addresses medical needs through clinical assessment, non-pharmacological interventions, and medication management.
  • Nurse – Focuses on caregiving support, optimising care environments, caregiver skills, and nursing procedures tailored to each senior’s needs.
  • Social Worker – Supports psycho-emotional well-being and assists with financial aid, community resources, and care planning.

Palliative Care Support

When seniors and families face end-of-life transitions, Hua Mei Mobile Clinic walks alongside them with compassion. We also provide palliative care that prioritises comfort, peace, and dignity, supporting seniors to remain in their preferred place of care, surrounded by those who matter most.

Our Commitment

We believe in long-term partnerships, not one-off visits. Our team works closely with seniors, caregivers, and families to deliver coordinated, continuous care at home.

What We Offer

  • Home visits by doctors, nurses, and social workers
    • Early intervention for acute health issues (for seniors known to us)
    • Long-term medical follow-up and prescription of chronic medications
    • Nursing services such as wound care, tube changes, and caregiver training
    • Home vaccination and phlebotomy (when indicated and for seniors known to us)
  • Care coordination, including social support and linkage to community resources
  • Referrals and arrangements for counselling, hospital specialist reviews, and other services as needed
  • Caregiver training and education

Who We Serve

  • Older persons aged 60 and above who are homebound or have significant difficulty travelling for medical care
  • Residents of Alexandra, Bukit Merah, Chinatown, Commonwealth, Ghim Moh, Outram, Queenstown, Tanjong Pagar, Telok Blangah, Tiong Bahru, Whampoa/Bendemeer
  • Older persons living outside these areas may be considered on a case-by-case basis

  • Self-referral via our enquiry contacts below
  • Fees: services are chargeable and eligible subsidies based on the National Household income-based Means Testing System would apply.
