This course equips you with the essential foundation to deliver effective, person-centred care for older adults. You will gain a clear understanding of the ageing process, explore key principles of quality care, and learn how to support older adults while working effectively within Active Ageing Centres. Discover the roles and responsibilities of Care Coordinators, Programme Executives, and Assistant Care Managers to help you deliver coordinated, meaningful, and impactful care.
This course is suitable for you if you are:
- Community care professionals supporting older adults, including Care Coordinators, Assistant Care Managers, Programme Executives/Officers, and support care staff in Active Ageing Centres
- Healthcare and social service professionals involved in eldercare, such as nurses, allied health professionals, and social workers
- Volunteers and family caregivers who want to build foundational knowledge in caring for older adults
By the end of this course, you will be able to:
- Understand the principles and concepts of effective care delivery for older adults
- Identify older adults’ needs by understanding the ageing process and its trajectory
- Recognise the different aspects of ageing and their implications on care
- Explain the roles and responsibilities within Active Ageing Centres to support coordinated, person-centred care.
